Ltd. is currently seeking a dedicated Medical Social Worker to provide essential psychosocial support to patients and their families within a Nursing Home environment. In this impactful role, you will work closely with medical professionals to ensure holistic patient care, with a specific focus on Palliative Care services.
You will assess the emotional and social needs of residents, coordinate care plans, and act as a vital link between patients, families, and community resources. Tanggung Jawab
- Conduct thorough psychosocial assessments for patients in the Nursing Home setting to determine their needs and support systems.
- Develop and implement individualized care plans in collaboration with the medical, nursing, and therapy teams.
- Provide emotional counseling and support to patients and families, particularly during critical transitions and end-of-life (Palliative Care) situations.
- Coordinate discharge planning and connect patients/families with appropriate community resources and social welfare programs.
- Advocate for the rights and best interests of patients within the healthcare system.
- Facilitate support groups and family meetings to improve communication and coping strategies.
- Document all patient interactions, assessments, and care plans accurately in electronic medical records.
Kualifikasi
- Degree in Social Work (BSW) or Master of Social Work (MSW) is required.
- Minimum of 2-3 years of relevant experience in a healthcare, hospital, or community social work setting.
- Strong experience and knowledge in Palliative Care and geriatric care is highly desirable.
- Deep understanding of community resources, government subsidies, and social welfare schemes in Singapore.
- Excellent communication, interpersonal, and active listening skills.
- Ability to work independently as well as effectively within a multidisciplinary team.
- Valid professional social work license.
