Tanggung Jawab
- Assess patients' psychosocial needs and develop tailored support plans
- Facilitate financial assistance applications for vulnerable patients
- Provide emotional counseling and crisis intervention to patients/families
- Collaborate with multidisciplinary healthcare teams for integrated care
- Maintain accurate case records and documentation
- Educate patients on community resources and social services
- Conduct home visits to evaluate living conditions and support needs
Kualifikasi
- Diploma in Social Work, Psychology, or related healthcare field
- Minimum 1 year experience in healthcare or social services
- Certification in counseling or crisis intervention preferred
- Strong interpersonal and communication skills
- Ability to handle sensitive situations with discretion
- Proficiency in healthcare documentation systems
- Knowledge of Singapore's social assistance programs
