- Coordinate and manage patient care plans in alignment with primary care objectives.

Kualifikasi
- Diploma or Degree in Nursing, Allied Health, Social Work, or a related healthcare field.
- Minimum of 2-3 years of experience in care coordination, patient services, or community care.
- Strong interpersonal and communication skills with a patient-centric approach.
- Ability to work independently and manage multiple priorities effectively.
- Knowledge of Singapore's healthcare landscape and primary care protocols is highly desirable.
- Proficiency in using electronic health records (EHR) systems.
- Compassionate demeanor with a strong focus on patient advocacy.
